Make sure of the following:

  1. mod_rewrite is loaded; and
  2. there is a .htaccess file in the root directory of the installation.

on the server, your sure all the folders have the correct permissions?

you also may have to go to the Admin>Flash Apps and click the pluses on each module and make sure they are installed, especially if this is only happening on media modules.

Nathan, his php info shows it installed

You may find this old thread helpful in getting Dolphin set up to run on Ubuntu,

http://www.boonex.com/forums/?action=goto&my_flags=1#topic/D7-Ubuntu-9-10.htm

it is the one I used to get my home server running, http://www.duvallocals.info/

the photos are missing from the thread but your server is already running so make sure your have ffmpeg installed correct too.

If it's loaded (and he really does have full control of the server, suggesting it's VPS or dedicated), it might still be the second option.  It's also possible he's using a default Apache configuration, which doesn't allow the use of .htaccess files.  If that's the case, he needs to edit the site configuration file (/etc/apache2/sites-available/default) and change AllowOveride from "None" to "All."
