6.1.5 issue?

I just installed a fresh copy of 6.1.5 onto my new server and things are going well, except for editing membership levels of new members.  Here is what I am seeing:

I have tried recompiling the language file but that isn't working.  Any ideas?

In fields builder, look at those fields in admin view and make sure they have a caption.  I'm pretty sure that is all that is.

I checked that and noticed that they do have a caption. I remember seeing this one time in 6.1.4 and I managed to fix it myself but darn if it hasn't escaped my memory.

Anybody else?  I am racking my brain against this one.

Admin Dashboard > Settings > Language Settings > compile language file to refresh your Caption boxes correctly.

Just tried that.

Still not working right.  I don't think it is the language settings.

This is the wierdest thing I have seen.  I even prowled the database settings and they look good as well.

So this is a snap of what it looks like when I expand the language file from the admin panel:

I just did a fresh test install on the same server, went through and created a test account, and of course that is working just fine.  I did an export of the language database files and imported them into the actual database, no effect.

I can do a reinstall on the root server to fix this, but before I do, anyone else have any ideas what might have gone awry?

Fresh install of the original 6.1.5 full was done yesterday which is the problematic install.  Could there have been a fluke of a bad file somewhere uploaded???
