Activate account before login

If user does not activate his/her account, they are able to login under inactive user's status and browse the site. I want users to activate their account by email, before they login.

I want to know too!  I'm still looking, but I want to deny users access to the site unless they've been approved by an administrator.  The community I'm building is for a psychotherapist, and privacy is of the utmost importance!
