Advanced Radio Buttons

I would like to extend those of radio buttons and obtain the following result:

Does someone have an hint for me, how I can represent the fields like above?

Including the designations: properly and chaotically?

I had hoped to find a possibility where I could use for several character fields same 'Predefined Values'.

Without putting down the character description with in 'Values'

Maybe this is possible in dolphin 7? :)
