Changing affiliate signup page

Is it possible to change the affiliate signup page so that members enter their first and last name instead of a nickname (there is only one field available using 2-10 letters).  Where would I do that and by changing any of this info, would it mess up the rest of the files?

Is there a MOD that will add AFFILIATE SIGN UP to the JOIN PAGE?.... maybe just a check box.

I've added FIRST NAME, LAST NAME and MAILING ADDRESS fields (for commision checks) on the JOIN PAGE and since E-mail and a Password are already requested on the JOIN PAGE, this information can be used for Affiliate Information.

If this is not available, how can I make the RESULTING page after joining, go to the AFFILIATE SIGN-UP page?
