Dropdown Box Issue

Hi there,

I am trying to put a drop down box on my join form.  the box exists but when I go to enter the fields all of them disappear except 1 selection.  Why is this happening?

Also How do I enter a blank space in a drop down box.  Say the member doesnt want to fill out the dropdown box.
