Feedback mod doesn't display

Here is a head scratcher - I think I have had this before also. That feedback form simply doesn't appear on my site, though the block is right their in the builder on the homepage.  What am I missing?

So have people stopped participating here entirely?  I am not having the same issue with the "Wall" - it simply isn't displaying on the site.  Anyone have anyclue as to why?
