HELP JOIN SITE ERROR!!!

Could someone please tell me how i can fix my join form. I tried to install a module from the market. It was to get your join form to be on your home page. But it did not work for me so i tried to change it back and when someone tries to join it says ERROR Database Query error. the mod  you tried to install was did it require you to edit files on this account? if so you just need to read verbatim what was changed, the other thing as a suggestion, if you ever need to edit a file, you would benefit best by copying the file and rename it with something like orig as a prefix or suffix to the file name, so if you need to revert to that original file you will have it as a backup.

if this was a mod that did not require you to edit the source code, then i would make every attempt to contact the author of the mod and have them help out getting you squared away.

just need more information on this, so it can be addressed correctly.
