How to Change the Search Fields

I would like to be able to change the search fields , Specific and Options

Our board is a niche for users who have skin conditions

What we would like to do is similar to  this site:

http://psoriasissocial.org/search.php

Where the Specific options have changed as well as the Options now have selections of fields that are perhaps chosen during signup or updated by the users profiles and such. 

Is there a 3rd party / extension - module that makes these types of changes easy?
