Need Help, populating a select field from a table on a form!

I need to know what files are needed to use a select field.

I already have a table setup for the list, and I can create an actionTest() in my module and I get  if I go

to /m/sponsors/test  I get a dropdown box that's populated.

Can I call on the selector field from a form, and populate it from a MySQL Table or the sys_pre_values table?

I want  a drop down box filled in with values on a form in my mod.  In my mod there's a sponsor's form to fill out.

Business Id- Text field  Category- Selector  and Sponsor Level :text          First picture on first post.

For the category field  I need a drop down box, select, or selector that I can fill with the information from a table. Picture 2 and 3.

Picture 3. Is a drop down I created in an actionTest() function on my bdSponModule.php.  I can't get it to work in a form though. just on it's own. The screenshot is by calling   ... m/sponsor/test
