Payment for membership

This seems to be a pretty basic task but for some reason I am stumbling...can anyone help? I want to have two levels of membership and charge a fee for the one level. Can anyone give me pointers on how to do this? Also the Create section.....is this to create a new membership level? If so why is it requiring an icon associated with it? Feel stupid but would appreciate any pointers! :)
