Payment system not upgrading members

The payment system is not auto upgrading members when they complete payment. I am using Igor's CCBill mod as payment processor which uses dolphin's payment system. The payments are being logged ok both in admin and in user's account, but their membership level remains the same. I am guessing it's the dolphin payment system that handles the membership upgrades rather than the ccbill mod. Any ideas?

OK this only applies to promotional members. What gives?
