Report a member option????

how can i set up a 'report a member' setting on my site, i want it ike the 'report spam' setting on the profiles, but i want so other members have the option to report that certain member for a certain reason like, abuse, miss use of site, etc, but i woud need a option so that member could let admin know the reason why they ave reported that member

surely this optionshould be standard on al ommunity websites shouldnt it? with all tha went on with facebook, there should be  report a member option, or all the weirdos out there would have a field day :s
