Youtube embed error

Youtube embed was working fine, I updated to the final and all of a sudden it has stopped. Any ideas?

I also have same error. i enter the url then it process then show edit info tab i edit it and press submit then it say VIDEO UPLOAD ERROR (FAILED TO ACCEPT INFORMATION)

Please Help me when i entered all info and press submit it do nothing but in flash/modules/videos/files have id_temp_small.jpg then it not rename and not executing INSERT INTO command in my iis hosting why ????????????????????????
