This could be a problem with URL rewrite engine. Please chech that the .htaccess file is on the root of your Dolphin installation. Then go to your admin panel - settings - permalinks and check that Enable friendly permalinks in avatars: is checked. If everything is ok as I stated, then you have a problem in your apache configuration and you should contact your hosting provider or simply disable the permalink for the avatar module. Does your host allow you to enable mod rewrite?
Dolphin depends on it for its friendly permalinks. Until you get those set up, you can browse to your admin panel and under Settings look for Permalinks. Uncheck "Enable friendly ..." for each module you are using.
